Description The aim of this module is to support advancement of clinical competence and knowledge related to Chronic Disease Management. The module will provide the student with an in-depth view of the needs of people living with chronic disease. Students will be equipped with the knowledge and skills required to develop and implement comprehensive assessment tools, and engage in comprehensive assessment and complex care planning for the prevention and management of chronic diseases, including cardiovascular diseases, respiratory diseases and diabetes. | |||||||||||||||||||||||||||||||||||||||||||
Learning Outcomes 1. LO1 Examine the pathophysiology of prominent chronic diseases, and their relationship with altered health, multi-morbidity and co-morbidity. 2. LO2 Apply the principles of comprehensive assessment to the prevention and management of chronic disease. 3. LO3 Organise a comprehensive, integrated plan of care to address the key needs of people living with chronic diseases, underpinned by the principles of chronic disease management, supportive care and palliative care. 4. LO4 Critically evaluate contemporary non-pharmacological and pharmacological and approaches to managing chronic disease. 5. LO5 Identify a range of evidence-based interventions to support family/non-professional caregivers. 6. LO6 Evaluate the appropriateness of care delivery methods and plans in the context of chronic disease management, and their application within specific cohorts of the Irish population. | |||||||||||||||||||||||||||||||||||||||||||
