Description Upon successful completion of the practice based module of study, a student will be able to demonstrate: Professional role enhancement in the management of chronic disease in collaboration with the multidisciplinary team and in partnership with people living with chronic disease and their families. Advanced clinical skills on general physical assessment and intervention regimes for individuals with CDM. An ability to apply nursing specialist knowledge of CDM (e.g. disease processes, assessment techniques, interventions. Including behavioural change theories) relating to CVD, Respiratory Diseases , Stroke and Diabetes , and individuals with reduced cognitive ability demonstrating impact on related patient care outcomes (PRO) and CDM pathways of care. An ability to inform patient care and at a local and national level influence services development initiatives based on related policy agendas. The student will have attained an ability to pursue relevant generalist /specialist pathways within the nursing workforce. Demonstrating understanding and skills to interpret and diagnose health issues through defined care planning, lifelong learning and flexibility to sustain personal and professional development to address CDM needs in specific population groups as they evolve. | |||||||||||||||||||||||||||||||||||||||||||
Learning Outcomes 1. Demonstrate insight into their own and others health behaviours and the impact of the learning to the highest ethical standards of clinical practice. 2. Recognise, understand and respect the complexity of sociocultural and international diversity on health behaviours and outcomes relating to CDM and specific diseases such as CVD, Respiratory Diseases, Stroke and Diabetes 3. Demonstrate professional competency on physical assessment and interventions and the capacity to interact effectively and sensitively with people across the chronic disease spectrum, across health care sectors, from diverse backgrounds and cultural perspectives understand and recognise inequalities in health and act accordingly; 4. Through verbal and written communication illustrate challenge based learning and an in depth understanding of CDM knowledge, clinical practice models and the associated concepts and theory that supports service based delivery of CDM as part of an inter-professional team 5. To demonstrate CDM nursing practice in context of service delivery and be able to put in place digital data and health literacy skills and to plan/design/and evaluation process as per programme pathway defined Entrustable Professional Activity Milestones on a selected case study relating to CVD, Respiratory Diseases , Obesity, Reduced cognitive ability and Diabetes | |||||||||||||||||||||||||||||||||||||||||||
