Description
The module will enable students to explore the principles and practice of primary health care, community health nursing and population health with a focus on the care of the older person. End of life care will also be explored both in community and acute health care setting.

Learning Outcomes
1. Debate the principles and practice of primary health care 2. Examine the concept of community and its relevance to understanding health care delivery 3. Debate the principles and practice of community nursing and its relevance to individual, family and community health 4. Discuss the principles of population health with a focus on the older person 5. Identify the scope of end of life nursing practice and explore its unique characteristics in relation to caring for people with life limiting and terminal illness in all settings. 6. Discuss the provision of appropriate, evidence-based nursing and supportive care to those at the end of life, their carers and families.

Community nursing and approaches to care throughout the life cycle
The principles and practice of primary health care.
Inequalities in health.
Healthcare structures, multidisciplinary team working and intersectoral collaboration
National and international health care policy
Needs assessment
Evolution, philosophy & Principles of End of Life Care. Exploration of the history of and the principles underlying end of life care. Cultural considerations in death and dying are also considered
Impact of Life-limiting illness: Physical, Psychological, Social and Emotional Factors Exploration of the impact of a diagnosis of life limiting or terminal illness on the individual and their family including the array of emotional responses that can occur, the enormity of the adaptive process and the relationship between psychological distress and physical suffering.
Care of the Actively Dying Patient Management of the complex symptoms including: pain, dyspnoea, delirium, restlessness and agitation. Ethical issues in end of life care.
