Integrated care
The principles of illness prevention, patient empowerment, multi-disciplinary cross-service care planning and delivery, where all health and social care services work together to provide a flexible network of care responsive to the changing needs of patients and their families. Thinking about the inputs of public and private providers, patient groups, clinicians and the voluntary sector – indeed, all healthcare stakeholders.

Leadership and Change Management
Change management and leadership; quality improvement approaches, models and frameworks. Describing the change, making the arguments, financial and strategic goals. the characteristics of transformational leadership and its effects on groups. creating environments where change is seen as a positive benefit to all. Supporting teams with empathy and optimism, making and embedding change. Lewin’s change model and Kotter’s 8 step theory.

Person and public engagement
Patient and public involvement in service development; stakeholder engagement in service development

Nursing role in the development of integrated care
Nursing role in developing integrated care services; Multidisciplinary team roles in development of integrated care services. Role of digital health and technology in integrated care

Service evaluation
Case studies in ANP/CNS-Led services (pathway specific) Service evaluation, quality outcomes and metrics in nurse-led services. Research utilization / Evidence based practice models and how they are applied to clinical practice. Evaluating health interventions, evaluation tools and concepts.
