Learning Outcomes 1. Display an in-depth understanding of integrated models of care for the clinical population they serve. 2. Analyze the opportunities and challenges of integrated care models for improving patient health and well-being within the chosen pathway 3. Develop leadership skills to support the advancement of practice related to their clinical population 4. Articulate the nursing role in change management in planning, developing, and implementing high-quality integrated care services for the relevant clinical population. 5. Critique the impact of cultural, political, environmental, and socioeconomic factors on change management and the development of quality integrated care services for the relevant clinical population 6. Integrated care The principles of illness prevention, patient empowerment, multi-disciplinary cross-service care planning and delivery, where all health and social care services work together to provide a flexible network of care responsive to the changing needs of patients and their families. Thinking about the inputs of public and private providers, patient groups, clinicians and the voluntary sector – indeed, all healthcare stakeholders. Policy National and international policies Leadership and Change Management Change management and leadership; quality improvement approaches, models and frameworks. Describing the change, making the arguments, financial and strategic goals. the characteristics of transformational leadership and its effects on groups. creating environments where change is seen as a positive benefit to all. Supporting teams with empathy and optimism, making and embedding change. Lewin’s change model and Kotter’s 8 step theory. Person and public engagement Patient and public involvement in service development; stakeholder engagement in service development Nursing role in the development of integrated care Nursing role in developing integrated care services; Multidisciplinary team roles in development of integrated care services.
