Description

The aim of this module is to provide specialist theory and purposefully designed learning activities to support the Chronic Disease Management Pathway designed to assist students to extend their practice in the care of patients with Chronic Disease Management. To advance the students ability for informed clinical judgement using supporting evidence based care in critical decision making for targeted patient centered CDM support. Students will be provided with the skills, competencies and capabilities to practice independently and contribute to the implementation of the Slaintecare Implementation Plan at the service level as part of an interdisciplinary team. Competency and skills will include introduction to targeted resources for self-management support interventions in partnership with people living with chronic disease and their families. The students will learn to facilitate patient / service user engagement and adopt targeted education for healthy lifestyle choices and patient safety. Including challenge based learning activities, peer to peer learning, and digital literacy skills

Learning Outcomes

1. The student will construct a logical progression of specialist clinical practice capability and competency for management of chronic disease. Understand related concepts of multi-morbidity and co-morbidity and illustrate core skills in related topics such as patient safety, medication management, clinical audit, appraisal of appropriate evidence and knowledge generation for care planning using key characteristics to deliver best practice in CDM.
2. 2. The student will organise and plan the extension of their practice with supporting evidence in critical decision making for targeted patient centred CDM. Advancing the Slaintecare Implementation Plan at the service level with targeted use of digital health technology such as telehealth consultations. For example, supporting self-management interventions in partnership with people living with chronic disease and their families including those individuals with redacted cognitive abilities and obesity targeting behavioural modifications education and healthy lifestyle choices.
3. The student will formulate leadership skills: specifically a respect for, and use of, critical and creative thinking, scholarly inquiry including challenge based learning, political awareness and the scientific approach to solving problems in CDM. Professional competencies include established effective critical thinking skills; engagement in creative thinking; appraisal for reasoning to recognise, develop, defend and criticise arguments and other persuasive appeals; an ability to approach clinical problems and care effectively through peer to peer learning
4. The student will organise and plan quality improvement research and evidence based practice at the organisational level. Construct and plan management strategies in a variety of contexts to measure effectiveness and impact in various formats and for various purposes . Examples of contexts include clinical, domestic, acute and secondary care.
5. The student will evaluate clinical practice models to extend practice, skills and competencies to illustrate a range of clinical skills in the assessment, diagnosis and safe interventions for people with CDM. Using a range of interpersonal, behavioural and communication skills in the support of people on coherent self-management programmes.
6. The student will evaluate the appropriateness of care delivery methods and plans. This will include interpreting the impact of the independent nursing contribution and as part of an interdisciplinary team to advance continuity of care with a dedicated case study relating to CDM
